If you require warranty replacement, please be sure to describe your issue and include following information:
- Your Crowd Supply or SlimeVR Store order number
- The issue you have (Blue Light of Death, broken extension cable or case, or anything else)
- Your shipping address, including name and phone
- Make sure your email is correct, or we won't be able to contact you!
